- Patient Information Sheet
- Patient History
- Patient Medication List
- Patient HIPAA Agreement
- Insurance cards
- Prescription cards
- Photo identification
- Any medical records from other doctors or facilities
Things to know:
Insurance Patients:
All co-pays and deductibles must be paid in full at the time of service.
Self-pay Patients:
Minimum of $250 is due on the first visit. For the remaining balance, please set up a payment agreement with receptionist at check-in.
